FY2015 Health/Dental Insurance Summary Memo                                                  Online enrollment instructions

If you are making NO changes to your current enrollment, there is nothing you need to do to continue your coverage for FY 2015. Employees need to do an online enrollment under the following conditions: changing plans; adding/dropping dependents; contributing to a flexible spending account; declining health insurance coverage (even if you currently decline the coverage).

For a complete schedule of benefits for each plan and important notices about your health insurance coverage, click on the appropriate link below. Also, for those enrolling in the High Deductible Plan, the designated Health Savings Account form below must be completed and sent to the HR office.

In compliance with the Affordable Care Act, the College has provided a Summary of Benefits and Coverage for each plan offered. This document, in a standard format, summarizes important information about each health coverage option to help you compare across options. Contact HR if you would like a paper copy. Please refer to the provided Schedule of Benefits above for specific plan information.
